Expedited no-fee passport information Published April 24, 2011 By 35th Fighter Wing Public Affairs MISAWA AIR BASE, Japan -- The last chartered flight under the voluntary departure plan for family members to return to Misawa left the Seattle/Tacoma Airport early 24 April destined for Misawa Air Base. All remaining family members will now be required to book their return flights through the Scheduled Air Transportation Office for routine Patriot Express or other commercial booking options. No matter what method of travel is used, military family members, to include infants, are required to have passports to enter Japan. Family members should visit the nearest military installation and meet with the passport agent to obtain an expedited no-fee passport. No-fee official passports are issued by the State Department's Special Issuance Agency. The SIA has made special allowances and exceptions by waiving the requirement to provide the child's birth certificate to accommodate family members in this predicament. However, other documents as outlined below are required in place of the child's birth certificate. The passport agent will complete the application form (DD Form 1056). Also required are the following documents: 1. Evacuation orders. 2. Proof of citizenship for both parents. For the sponsor in Japan, a copy of the birth certificate or passport will suffice. For the spouse in CONUS the passport will suffice. 3. A notarized consent signed by the sponsor in Japan to allow the CONUS spouse to process the DD Form 1056. 4. A passport photo of the child. 5. Copy of the marriage certificate. If a military installation is not accessible, the spouse may accomplish the above at any U.S. Post Office, but a $25 processing fee will be charged. Applicants are asked to remain patient with MPO passport representatives, because they may not be familiar with this exception since this is a special circumstance.
